Pharmacist Collaboration in the Care of Patients with Diabetes within the Primary Care Clinic Setting

About this study

The primary aim of this study is to compare the outcome measures of adult ECH type 2 diabetes patients who were referred to onsite pharmacist services for management of their diabetes to similar patients who were not referred for pharmacy service management of their diabetes. A secondary aim of the study is to assess the Kasson providers’ satisfaction level and estimated pharmacy service referral frequency to their patients. A tertiary aim of the study is to compare the hospitalization rates of type 2 diabetes rates who were referred to onsite pharmacist services for management of their diabetes to similar patients who were not referred for pharmacy service management of their diabetes.

Participation eligibility

Participant eligibility includes age, gender, type and stage of disease, and previous treatments or health concerns. Guidelines differ from study to study, and identify who can or cannot participate. There is no guarantee that every individual who qualifies and wants to participate in a trial will be enrolled. Contact the study team to discuss study eligibility and potential participation.

Inclusion Criteria:

  • Research authorized
  • Adult patients (ages 18 and over)
  • Patients empanelled to the Mayo ECH practices in Rochester and Kasson, Minnesota
  • Patients with a diagnosis of Type 2 Diabetes Mellitus and an A1c reading greater than 8% recorded from April 1, 2012-June 30, 2016.

Exclusion Criteria:

  • Pregnancy (within the study time period April 2012 through September 2016)
  • Baseline A1c
  • Referral to Endocrinology in month prior to pharmacist referral
