About this study
The objectives of this pilot study are 1) to assess the feasibility and effectiveness of integrating the Stanford Chronic Disease Self-Management Program (CDSMP) into the Mayo Clinic Dan Abraham Healthy Living Center (DAHLC)’s current structure and, 2) to explore the factors that might account for gender differences in the effectiveness or applicability of the program
Participation eligibility
Participant eligibility includes age, gender, type and stage of disease, and previous treatments or health concerns. Guidelines differ from study to study, and identify who can or cannot participate. There is no guarantee that every individual who qualifies and wants to participate in a trial will be enrolled. Contact the study team to discuss study eligibility and potential participation.
Inclusion Criteria:
- Are an adult (≥ 18 years) with the diagnosis of at least one chronic disease
- DAHLC members
- Do not have major barriers to provide written informed consent and able to participate (i.e., severe vision or hearing impairment, cognitive impairment, non-English speaking)
Exclusion Criteria:
- Unable to provide consent
- Do not have the diagnosis of at least one chronic disease
- Not DAHLC members
