A Study to Compare Telemonitoring versus Usual Care for High Risk Elderly Patients

About this study

The purpose of this study is to determine the effectiveness of home telemonitoring compared to usual care in reducing the combined outcomes of hospitalization and emergency room visits in a high risk population of patients over 60 years of age with multiple chronic illnesses.

 

 

Participation eligibility

Participant eligibility includes age, gender, type and stage of disease, and previous treatments or health concerns. Guidelines differ from study to study, and identify who can or cannot participate. There is no guarantee that every individual who qualifies and wants to participate in a trial will be enrolled. Contact the study team to discuss study eligibility and potential participation.

Inclusion Criteria

  • Age at least 60 years
  • An Elder Risk Assessment (ERA) Index score of 16 or greater
  • Able to participate fully in all aspects of the study
  • Provided with, understand, and signed the informed consent

Exclusion Criteria

  • Currently residing in a nursing home
  • Has a clinical diagnosis of dementia
  • A score of ≤29 on the Kokmen short test of mental status
  • Cannot give informed consent
  • Under age 60
  • Has not given Universal Research Authorization to use medical records
  • Not able to use the interventional machine
