Collaborative Approach to Resolving Food Insecurity and Enhancing Diabetes Outcomes

About this study

The purpose of this study is to assess the effectiveness of food prescriptions and education for patients ages 18-75 who are empaneled in the SWMN (Mankato, North Mankato, and St Peter) Family Medicine Clinics who have diabetes mellitus type 2 with hyperglycemia (A1c > 7) and have self-identified as experiencing food insecurity in the previous 6 months.

 

Participation eligibility

Participant eligibility includes age, gender, type and stage of disease, and previous treatments or health concerns. Guidelines differ from study to study, and identify who can or cannot participate. There is no guarantee that every individual who qualifies and wants to participate in a trial will be enrolled. Contact the study team to discuss study eligibility and potential participation.

Inclusion Criteria:

Interventions group:

  • Adult patients with DM2 with hyperglycemia (A1c > 7.0) as identified through EPIC medical records
  • Self-identified food insecurity through the EPIC Social Determinants of Health Questionnaires,
  • No significant dietary restrictions that would limit the use of the Food Rx.

Control group:

  • Usual DM2 care and

Exclusion Criteria:

 Patients who participated in the first version of the Food Rx study will not be offered participation in the study.
