Cold Snare EMR Trial

About this study

The primary aim of the study is to compare safety and effectiveness of two methods of endoscopic resection of large colorectal polyps: cold snare EMR (without electrocautery) vs. hot snare EMR (with electrocautery).

Participation eligibility

Participant eligibility includes age, gender, type and stage of disease, and previous treatments or health concerns. Guidelines differ from study to study, and identify who can or cannot participate. There is no guarantee that every individual who qualifies and wants to participate in a trial will be enrolled. Contact the study team to discuss study eligibility and potential participation.

Inclusion Criteria:

  • Any patient ≥ 18 years of age who presents for a colonoscopy and who does not have criteria for exclusion.
  • Patients with a ≥ 20mm non-pedunculated colorectal polyp.

Exclusion Criteria:

  • Pedunculated polyps (as defined by Paris Classification type Ip).
  • Suspected adenocarcinoma with deep submucosal invasion.
  • Patients with ulcerated depressed lesions (as defined by Paris Classification type III) or confirmed adenocarcinoma.
  • Patients with inflammatory bowel disease.
  • Patients who are receiving an emergency colonoscopy.
  • Poor general health (ASA class > 3).
  • Patients with coagulopathy with an elevated INR ≥ 1.5, or platelets < 50.
  • Inadequate bowel preparation (Boston Bowel Prep Scale, total score ≤ 2).
  • Pregnancy.
  • For patients who did not meet patient inclusion criteria, we will record the reason for ineligibility.
  • For consented and study subjects who did not meet polyp inclusion criteria, we will collect information on reasons for exclusion of polyps (including size, morphology, location, histology), how they were removed (hot or cold snare resection) and whether bleeding occurred during the resection.
