Weight and Wellness Solutions Registry Study

About this study

The purpose of this study is to establish a registry database of patients that will be utilized for future research studies related to obesity, health, and weight management strategies.

Participation eligibility

Participant eligibility includes age, gender, type and stage of disease, and previous treatments or health concerns. Guidelines differ from study to study, and identify who can or cannot participate. There is no guarantee that every individual who qualifies and wants to participate in a trial will be enrolled. Contact the study team to discuss study eligibility and potential participation.

Inclusion Criteria:

  1. Provision of Arizona authorization for the use of medical records for research
  2. Age 18 or older
  3. Consent to complete and update Weight and Wellness Solutions Program
  4. Signed consent    

Exclusion Criteria: 

  1. No Arizona authorization for medical record use for research
  2. Under age 18
