Instrument for Patient Capacity Assessment (ICAN) in Patients with Multiple Chronic Conditions

About this study

The purpose of this study is to implement the use of the Instrument for Patient Capacity Assessment (ICAN) discussion aid and describe its usefulness rising patients’ capacity domains of concern in comparison with a comprehensive nurse home visit capacity assessment. We also seek to explore the overall experience of physicians and patients using the Discussion Aid and the factors influencing the quantity and quality of the information obtained.  

Participation eligibility

Participant eligibility includes age, gender, type and stage of disease, and previous treatments or health concerns. Guidelines differ from study to study, and identify who can or cannot participate. There is no guarantee that every individual who qualifies and wants to participate in a trial will be enrolled. Contact the study team to discuss study eligibility and potential participation.

Inclusion Criteria:

  • Patients:
    • 18 years of age and older
    • Receive Public Health Nursing services through Olmsted County Public Health
    • Two or more chronic conditions
    • At least one upcoming medical appointment in the next four months at Mayo Clinic in Rochester, Minnesota
  • Physicians:
    • All physicians of patients who are deemed eligible based on the inclusion criteria above and who are currently employed at Mayo Clinic in Rochester, Minnesota

Exclusion Criteria:

  • Patients with barriers to consent, such as cognitive disabilities
  • Unable to consent in English
