Development of a Risk Index for Prediction of Health Outcomes and Healthcare Utilization

Overview

About this study

The overall objective of this application is to develop, test and implement a comprehensive socio-demographic and behavioral (SDBH) risk index to identify important predictors of health outcomes and health care utilization.

Participation eligibility

Participant eligibility includes age, gender, type and stage of disease, and previous treatments or health concerns. Guidelines differ from study to study, and identify who can or cannot participate. There is no guarantee that every individual who qualifies and wants to participate in a trial will be enrolled. Contact the study team to discuss study eligibility and potential participation.

Inclusion Criteria:

  • Adult patients (18 years of age and older) empaneled in Employee Community Health, Rochester, MN

Exclusion Criteria:

  • Patients who decline consent or do not have the ability to be able to complete the survey.
