A Study to Evaluate Patient Outcomes in Care Transition of Acute Kidney Injury (AKI) Survivors

About this study

The purpose of this study is to assess the ability of education and outpatient kidney care follow-up to improve patient knowledge about acute kidney injury (AKI).

 

 

 

Participation eligibility

Participant eligibility includes age, gender, type and stage of disease, and previous treatments or health concerns. Guidelines differ from study to study, and identify who can or cannot participate. There is no guarantee that every individual who qualifies and wants to participate in a trial will be enrolled. Contact the study team to discuss study eligibility and potential participation.

Inclusion Criteria:

  • KDIGO Stage 3 AKI, PCP @ Mayo Rochester.

Exclusion Criteria:

  • Patient with need for dialysis at discharge from hospital or hospice.
  • Enrolled in the Primary Care Transition Program.
  • Non-English speaking.
  • Cognitive impairment.
  • Dementia.
