Health and Wellness Coaching Impact in Chronic Kidney Disease Settings

About this study

The purpose of this research is to evaluate the person-centered care model that integrates health and wellness coaching to standard of care and its impact on patients and their care partners within this chronic kidney disease care settings.

 

Participation eligibility

Participant eligibility includes age, gender, type and stage of disease, and previous treatments or health concerns. Guidelines differ from study to study, and identify who can or cannot participate. There is no guarantee that every individual who qualifies and wants to participate in a trial will be enrolled. Contact the study team to discuss study eligibility and potential participation.

Inclusion Criteria:  

  • Adult patients with autosomal dominant polycystic kidney disease (ADPKD) of any gender, race, and ethnicity, with a predicted life expectancy of 12 months minimum and receiving care at MCF in the Nephrology clinic.
  • Informal (unpaid) adult CGs of their respective patients.
  • Proficiency in English language for all participants.
  • Willing to provide informed consent and complete all study arm-related activities, independently.

Exclusion Criteria:

  • GFR ≤ 15, on dialysis, and/or received kidney transplantation.
  • Pregnancy.
