About this study

Diabetes mellitus is a common condition which is defined by persistently high blood sugar levels. This is a frequent problem that is most commonly due to type 2 diabetes. However, it is now recognized that a small portion of the population with diabetes have an underlying problem with their pancreas, such as chronic pancreatitis or pancreatic cancer, as the cause of their diabetes. Currently, there is no test to identify the small number of patients who have diabetes caused by a primary problem with their pancreas.

The goal of this study is to develop a test to distinguish these various causes of diabetes. To accomplish this goal, we will enroll a range of patients. Most of the patients enrolled for these groups will have developed diabetes within the last 3 years, but we will also enroll a small number of patients with long-term diabetes and normal blood sugars for comparison.

About this study

Using stem cell derived intestinal epithelial cultures (enteroids) derived from obese (BMI> 30) patients and non-obese and metabolically normal patients (either post-bariatric surgery (BS) or BS-naïve with BMI < 25), dietary glucose absorption was measured. We identified that enteroids from obese patients were characterized by glucose hyper-absorption (~ 5 fold) compared to non-obese patients. Significant upregulation of major intestinal sugar transporters, including SGLT1, GLU2 and GLUT5 was responsible for hyper-absorptive phenotype and their pharmacologic inhibition significantly decreased glucose absorption. Importantly, we observed that enteroids from post-BS non-obese patients exhibited low dietary glucose absorption, indicating that altered glucose absorption is a potential mechanism for the immediate and clinically significant improvement in glucose homeostasis after BS, represented clinically by resolution of Type 2 Diabetes (T2D) within weeks to months of surgery. However, BS confers these benefits in only in ~ 60% of patients, indicating a desperate need to identify the cohort of patients likely to benefit from such an invasive operation.
Thus, our results and other published data led us to the current hypothesis: an intestinal glucose hyper-absorptive phenotype in obese patients may be a predictor for successful outcome of BS. Additionally, sugar transporters responsible for hyper-absorptive phenotype represent potential targets for future drugs that could provide an alternative approach to BS.

About this study

The purpose of this study is to track the treatment burden experienced by patients living with Type 2 Diabetes Mellitus (T2DM) experience as they work to manage their illness in the context of social distancing measures.

To promote social distancing during the COVID-19 pandemic, health care institutions around the world have rapidly expanded their use of telemedicine to replace in-office appointments where possible.1 For patients with diabetes, who spend considerable time and energy engaging with various components of the health care system,2,3 this unexpected and abrupt transition to virtual health care may signal significant changes to the treatment burden associated with diabetes management.4 Previous research has characterized the effect of telemedicine approaches on outcomes such as glycemic control, patient satisfaction, cost savings, and appointment adherence rates,5-8 but its influence on treatment burden has not been explored. Importantly, work in this area has largely drawn from the experiences of patients who voluntarily selected to use virtual health services. The current situation, in which patients with chronic health conditions have been obligated to use telemedicine in order to receive care, therefore presents a unique opportunity to explore the ways in which virtual diabetes management impacts the workload associated with managing chronic illness.

Aim: Explore patient experiences managing diabetes mellitus in the context of recent widespread changes in healthcare and social distancing due to the novel coronavirus pandemic, including barriers and facilitators to obtaining needed health care services and changes in social networks. To accomplish this aim, we will conduct qualitative interviews with up to 30 participants.
